Transcriber's Notes
The file name gives the year as 1946, but the typed dates read 1945 throughout: 'February 1945' on the front, '2-28-45' in the button annotation, service '8-23-1943 to 2-28-1945', and 'MOP 2/28/45'. On this typewriter 5 and 6 look alike, but the flat-topped 5 is visible. The 1945 reading also agrees with the BuMed letter of 12 Jan 1945 and with a total net service of 1 year 6 months.
Total net service: the years figure is poorly struck and reads '1'. The days figure could be 5 or 6.
The service number reads '818 34 09'. The middle group might be '54'.
Vessel read as 'USS LCI(L) 1065'. The first letter is faint and could be taken for 'E'.
'PHIBTRABASE' is partly obscured at 'PH_B'.
In the personal marks, the item before 'r. index' reads 'S½"' (a letter S or 8 followed by ½ and a double-prime mark). Uncertain.
Officer names are uncertain. Executive officer: initials 'H. ?.' before 'CARROLL'. Medical officer: 'Z. J. WATERS' (initials unclear). Paying officer: 'H. C.' followed by an illegible surname. The medical officer in command's name, 'A. H. DEARING', is fairly clear.
The first typed line of the annotation at the top edge is cut off. Only fragments are visible, possibly 'UNITED STATES NAVAL HOSPITAL, ...'.
The back of the certificate is printed at 90° on the copy.
